Statement of Consent & Waiver
I, the undersigned, declare that I am the legal parent/guardian of the above-named child and hereby give my full consent for my child to participate in all scheduled summer camp activities held at the indoor playground facility.
I understand and acknowledge that participation in the described activities may involve inherent risks that could result in minor or serious physical injury. Nevertheless, I voluntarily permit my child to participate and accept full responsibility for any such risks.
In the event of a medical emergency, I authorize the camp supervisors and staff to take necessary action to ensure the safety and health of my child, including consenting to medical treatment or procedures as deemed appropriate.
I further agree that the camp management and its staff will not be held responsible for any injury, loss, or damage that may occur to my child in connection with camp activities, except in cases of gross negligence or intentional misconduct.
By signing below, I affirm that I have read and fully understand this waiver and consent form, and agree to the terms stated herein.
